42% of the Proceeds will go to Phoenix!

Proceeds will go the Phoenix Club Infrastructure Fund.

HOW TO ORDER:

ONLINE:

  • Go to https://danielchocolates.com/phoenix/
  • Browse through the entire selection on the <Christmas Collection> page with seasonal items and best seller’s assorted chocolates boxes.
  • Fill up your cart > checkout and enter the fundraising code PHOENIX2017
  • Select “Local Pickup” (no shipping charges will be applied) and then proceed to checkout.
  • Don't forget to write the Athlete's Name & Recreational/Team Program you are fundraising in behalf of in the Order Notes during payment process.

DOWNLOAD/PICK UP AN ORDER FORM:

  • Download/pick up an order form  from Millennium and St James lobby, fill in and send in to front desk.  
  • Each form must be accompanied by one payment that reflects the grand total amount for your form. (If you collect orders from friends and family, they pay you directly and you send one payment for the whole form to Phoenix.)

Order deadline: Sunday, December 3
Pick up date: Thursday, December 7
